Charitable Donation
A charitable donation request is one that is philanthropic in nature supporting the communities we serve. To request a donation, use the link below to create an account, upload a current and signed W9 form, and follow the remaining prompts. To be approved, your organization needs to be a 501c3 or a federally recognized non-profit organization with a valid W9 form.

How does my organization apply online for a donation request to my organization from Meijer?
If you are a 501(c)(3) or another federally recognized charitable organization, click the link under the Charitable Donation header. If you are are a for-profit organizatoin requesting a sponsorship, please email Keri Tomac at keri.tomac@meijer.com or Robert Lajcaj at robert.lajcaj@meijer.com.
How quickly will I know if my donation request has been approved or declined?
When you apply for a donation from Meijer, you will receive a confirmation number in your email. Please allow 60 days for review. Once the review process is complete, you will receive a status update via phone or email.
What information will I need to apply for a donation?
You will need to upload a current and signed W9 form and documentation supporting the request's reason. This could be a letter, sponsorship proposal, impact report, etc.
I am a school, what information is required to request donation support?
As a school entity, you will need to upload a current and signed W9 and provide the National Center of Education Statistics (NCES) code. If you are unsure what that code is, there are links in the form to help navigate you. If at any time you need support, there is a link at the bottom of the page in the application (Need Support) to assist.
Can I track the status of a donation request I submitted?
If it has been 60 days, you can contact a Meijer store to check on the status of a submitted donation request.
If I receive an error message when applying for donation support, where do I get help?
If you receive an error message in your application, there is a link titled ‘Need Support’ that will assist you in the application process.
If my organization does not meet the criteria of support, can I still request a donation?
If your organization does not meet the criteria to receive a donation (e.g., not a 501(c)(3) or other federally recognized non-profit organization), you will not be able to receive a donation from Meijer.
I work for multiple non-profits; do I need to create a new account for each organization?
No, you will only need to create one account for yourself and you can add multiple organizations to your account.
Can there be multiple payees (contacts) for a single organization?
Yes, an organization can have multiple contact names associated with their organization. (Example, schools, churches). The contact must have a current W9 and be a representative from that organization.
By setting up an account to receive donation support, will this tie into my mPerks account?
No, the donation support request form and mPerks are two separate programs.
Can Capital City Market enter in the request for donation support on my behalf?
Yes, a designated Meijer team member can add your request information on your behalf, but to ensure accurate information is added, we strongly encourage you to complete the application personally. Please contact Keri Tomac at keri.tomac@meijer.com if you need assistance.
May I request a donation from more than one Meijer location?
Please submit the request for your event to a single Meijer location. To help ensure we can support many organizations and causes throughout our six-state footprint, it is our policy to provide one donation per event.
